Home Place Homeschool Association (HPHA) Membership Requirements To maintain your HPHA membership, the following items must be completed and submitted on time: Required Submissions ✅ 90-Day Questionnaire & Progress Report Submit by March 1st Confirms 90 days of attendance Include a semi-annual progress report for your records (Progress reports are NOT submitted to HPHA) ✅ 180-Day Questionnaire & Progress Report Submit by July 31st Confirms 180 days of attendance Include an end-of-year progress report for your records (Progress reports are NOT submitted to HPHA) 🔔 Notify HPHA of any changes, including: -Address -Email -Phone number -Enrollment status State-Required Records (For Your Records Only) These documents are NOT submitted to HPHA but must be kept on file and up to date. Subjects to Cover (Minimum Requirement): -Math -Reading / Literature -Writing / Composition -Science -Social Studies Documentation to Keep: 📄 90-Day Progress Report (Semi-annual) 📄 180-Day Progress Report (End of Year) ✏️ Samples of Student Work in each required subject 📅 Attendance Records (Minimum of 180 days completed) 📚 Lesson Plans, Diaries, Plan Books, or other records documenting instruction in: -Reading -Writing -Math -Science -Social Studies (For grades 7–12: include Literature and Composition specifically) 🔍 HPHA reserves the right to conduct a records check at any point during the school year. Please ensure your records are accurate and up to date.
